Vivian "Kay" Phair, 67, Hope, died at 8:02 p.m. Monday, March 25 at Columbus Regional Hospital.
She was born Nov. 19, 1945 in Columbus, the daughter of Harry and Elizabeth Knotts Fish.
Ms. Phair retired as a case manager for Aging and Community Services.
Survivors include one daughter: Michelle (Todd) Fortini, Columbus; her mother; and her companion: Cathy Case, Hope.
Her father preceded her in death.
Funeral services will be conducted 1 p.m. Friday, March 29 at Myers-Reed Chapel, 3729 25th St., Columbus, with the Rev. Michael Hogg officiating. Burial will follow at Garland Brook Cemetery, Columbus.
Friends may call from 5 to 7 p.m. Thursday, March 28 and from noon to time of the service on Friday.
Memorials may be made to Simon Cancer Center IU Hospital.
