Isabella Ilene Cook
Lincoln- Isabella Ilene Cook, infant daughter of three days, died at 9:37 a.m. May 13, 2014 at St. John's Hospital, Springfield, IL.
Isabella was born May 10, 2014 in Springfield, Illinois, the daughter of Luis "Bubba" and Kasey Slack Cook.
Isabella is survived by her parents of Lincoln; two sisters: Alexandria Cook her twin and Sophia Cook; Maternal Grandparents: David and Sandy Slack of Lincoln; Paternal Grandmother: Heidi Cook of Lincoln; Maternal Aunts: Chelsie (Adam) Campbell of Lincoln; Jessie (Kyle) Clark of Lincoln and Paternal Aunts and Uncles: Leticia Cook of Lincoln; Alejandro Cook of Norwalk, IA; Chaka (Harley) Utterback of Lincoln; Allissa Martin of Lincoln and Cameron Cook of Lincoln. She was preceded in death by her Maternal Grandparents: Roy and Marjorie Slack; Don and Ilene Southern; Paternal Grandparents: Pete and Norma Moxley.
Private family services will be held Wednesday, May 21, 2014 with Rev. Mark Peters officiating. Services were entrusted to Fricke-Calvert-Schrader Funeral Home, Lincoln. Memorials may be given to the Ronald McDonald House.
