GLEN LYON — Gertrude G. Formulak, 92, of West Enterprise Street, Glen Lyon, passed away Thursday morning, Jan. 1, 2015, at Wilkes-Barre General Hospital.
Gertrude was born on March 5, 1922. She was a daughter of the late Edward and Agnes (Rich) Koshinski.
Gertrude was a graduate of Nanticoke High School. She was employed as a supervisor for American Tobacco until her retirement in 1978. She was a member of Holy Spirit Parish in Glen Lyon and was a member of its Altar and Rosary Society.
Gertrude was preceded in death by her husband of 63 years of marriage, Joseph S. Formulak; daughter, Gail Cregg; sisters, Mary Chepulis, Anna Spencer and Helen DeLuca; brothers, Anthony and Edward Koshinski.
Surviving are her daughter, Sharon Gayeski and her husband, Vincent, Griswold, Connecticut; sons, Gerald Formulak and his wife, Cynthia, Chino, California, Joseph P. Formulak, Glen Lyon; grandchildren, Michelle, Michael, Christopher and Renee; five great-grandchildren; several nieces and nephews; and her very special nephew, Joseph Lashinski.
The family thanks all of Gertrude's special angels who cared for her while she was at home. Their compassion and care was greatly appreciated.
Funeral services for Gertrude will be held at 11:30 a.m. Monday from the George A. Strish Inc. Funeral Home, 211 W. Main St., Glen Lyon. A Mass of Christian Burial will be held at noon in Holy Spirit Parish/St. Adalbert's Church, Glen Lyon. Interment will be held in the parish cemetery, Glen Lyon. Friends may call from 5 to 7 p.m. Sunday and 10:30 a.m. until the time of services Monday.

So Sorry to hear about Choci Gert ' s passing. My heart goes out to all of you. Just know she is in heaven and dearly loved by her family there. I will miss her she was a special lady.

I am so truly sorry for your loss. My prayers go out to your family at this time for peace of mind and comfort in your time of sorrow. I remember Aunt Gert's voice until this day. She was a strong and compassionate lady.
