Welch, David C., DO
David C. Welch, DO, 61, of Phoenix, AZ passed away suddenly February 2, 2014. David received his BA from Arizona State University and graduated from the West Virginia School of Osteopathic Medicine in 1983. His internship and residency in Emergency Medicine were spent at Brentwood Hospital near Cleveland, Ohio from 1983-1986. A third generation DO, David received his lifetime certification from the American Osteopathic Board of Emergency Medicine in 1992 and became a Fellow of the American College of Osteopathic Emergency Physicians in 1997. At the time of his death Dr. Welch served as Clinic Medical Director at Concentra West Clinic, Chief Medical Director for TriState CareFlight, and Medical Director for Wizard Education. Over his twenty-eight year practice David also served as full time emergency physician at John C. Lincoln - Deer Valley (1991-2006), formerly Phoenix General Hospital (1986-1989) and at Kingman Regional Medical Center (1989-2003). Always one who loved to share his knowledge, he founded and directed the AOA-approved Emergency Medicine Residency Program at Kingman Regional and was also a clinical professor at Midwestern University. Throughout his career Dr. Welch provided EMS Medical Directorship to multiple EMS and fire stations as well as Phoenix Emergency Rescue, and Wilderness Medics where he was also owner and president. Dr. Welch was also owner and president of Ultimate Urgent Care Centers and president of Phoenix Emergency Group, Inc. A highly respected emergency physician, David's genuine interest in people and great sense of humor endeared him to colleagues, co-workers, patients, and friends wherever he went. He will be deeply missed. David is preceded in death by his parents, Gloria D. Thomas and Dr. Donald S. Welch, DO. David is survived by his loving wife, Carol, children Jonathan (Angelita), Murphy, Adam, and Hannah; brother, Paul (Barbara) Welch, P.A. and sister, Judi (Rusty) Clarke; grandchildren Hannah and Nehemiah; and several nieces , nephews, and cousins. Visitation will be Friday, February 7, 2014 from 5-8 p.m. at Hansen Chapel, 8314 N. 7th Street, Phoenix. Funeral will be Saturday, February 8, 2014 at 10:00 a.m. at St. Paul's Catholic Church, 330 W. Coral Gables, Phoenix with burial to follow at Phoenix Memorial Park, 200 W. Beardsley Rd., Phoenix. Contributions may be made in his name to Midwestern University-Glendale Campus, Office of the President, 19555 N. 59th Avenue, Glendale, AZ 85308. Blessings of peace and grace for Dr. Welch and his Family. We were so blessed to be a part of their loving Family. Doc touched so many people and helped anyone in need. To Dr David Welch's family--I was shocked and saddened to hear of his death. I worked with him in the ER in Kingman for 10 years--He never passed up an opportunity to teach and he made emergency medicine an enjoyable occupation. I am so sorry for your loss. Carol and kids,
David was a cousin to me by marriage. How lucky we were! Knew and respected "Doc" for a long time, watched him flourish in practice, take care of his family and patients, enjoy them and
life. Spent time with David and the family at Bartlett Lake this past September, good times and lots of laughs. He will be missed, but always thought about and loved. The family was blessed to have him.
