Connie M. Chapman 1952-2012 PITTSFIELD Connie M. Chapman, 59, of 39 Day Street, passed away Wednesday March 7, 2012 at home comfortably and peacefully surrounded by her loving family. Born August 21, 1952 in Pittsfield, she was the daughter of Frederick Phelps and Beverly Jarrett, she attended local schools graduating from Taconic High School in 1970. Connie worked at the Mount Greylock Extended Care Facility as a CNA for 20 years until she became ill in May. Connie was a member of the Polish Falcons of America, Polish Community Club, She was an avid reader, loved to collect frogs and she loved to bake. She loved spending time with her family. She leaves her husband Stephen J. Chapman, whom she married August 21, 1991, her mother Beverly M. Jarrett of Pittsfield, her Father Frederick Phelps of Florida, Her sister Gail McGivern and her husband Bruce of Pittsfield, her brother Ronald Phelps and his wife Louise of Vermont, her Daughter Michelle Jones and her husband John of Pittsfield, her two stepsons Stephen Chapman and his wife Liz of Windsor, and Michael Chapman and his wife Tina of Florida, six grandchildren Kayla, John, Joe, Jeff, Jordyn and Matthew, many nieces nephews and Cousins, also her dog Kiley which was her best friend, and leaves many friends and coworkers. FUNERAL NOTICE: A funeral service for Connie M. Chapman will be held Monday March 12, 2012 at the WELLINGTON FUNERAL HOME, 220 EAST STREET, PITTSFIELD, MA 01201 at 11 a.m. with the Rev. Richard Hayes officiating, burial will be in the Pittsfield Cemetery. Calling hours will be held Sunday March 11, 2012 at the WELLINGTON FUNERAL HOME from 2 p.m. to 5 p.m. In lieu of flowers donations can be made to Hospice Care in the Berkshire in care of the funeral home.
