Zidek & Son Forest Glen Chapels, 5265 N. Elston Ave., Chicago, Sunday from 2 p.m. to 8 p.m. Lying In State at Trinity Lutheran Church, 5106 N. LaCrosse Ave., Chicago, IL 60630, Monday from 10 a.m. until time of funeral service at 11 a.m. Interment Memory Gardens Cemetery. In lieu of flowers, donations to Trinity Lutheran Church or the Slovak American Charitable Assn, 485 Newtown Dr., Buffalo Grove, IL 60089 and The Heartland Hospice, are appreciated. 773-777-2552.
