Tammy Rae Hart Cook, age 54, of Grimes, Iowa entered this life on December 7, 1958 in Madison, Indiana. She was the loving daughter of Raymond Fay Hart, Sr. and Shirley Ann Haskell Peddie. She also was raised by her father figure, Jon W. Peddie since the age of seven. She was raised in Madison and was a 1977 graduate of Madison Consolidated High School. She received her social worker degree from Ball State University. Tammy had worked at the Briar Patch with her mother as a server for several years. She had worked in social work all of her life having worked in Seymour and then for ten years at the Muscatatuck State Development in Butlerville, Indiana. She moved to Grimes, Iowa in 1994 returning to Madison with her mother's illness and death in 2012. Tammy had worked as a social worker and as the group home administrator at Crest Services in Des Moines, Iowa for twenty years. Tammy had attended the Lutheran Church of Hope in West Des Moines and after returning to Madison she attended North Madison Christian Church. Tammy found great joy in shopping, traveling, her animals, socializing with family and friends. Tammy found great pride in her appearance and enjoyed working out. Tammy always had a sparkle in her eye and a little bling in her outfit. She will be missed greatly by her devoted family and friends. Tammy died Thursday, March 7, 2013 at 7:40 p.m. at the Norton Hospital in Louisville, Kentucky.

Tammy will be missed by her loving son: Tyler Cook of Johnston, Iowa; her father: Raymond Fay Hart, Sr. of Hanover, Indiana; her father figure: Jon W. Peddie of Madison, Indiana; her loving sisters: Bonnie Hart of Madison, Indiana, Gina Jenkins and her husband, Mark of Madison, Indiana, Traci Peddie of Indianapolis, Indiana, Brandy Graham and her husband, Jimmy of Madison, Indiana, Mindi Brent and her husband, David of Campbellsburg, Kentucky, Carrie Hart and companion, Brook Reindollar of Madison, Indiana, Jayme Hart of Louisville, Kentucky; her brother: Raymond F. Hart, Jr. and his wife, Beth of Chicago, Illinois; her nieces and nephews: Brittin, Luc, Leigh Ann, Wade, Eric, Julia, Bralyn, Cory, Lucas, Andy & Jesse; her grandmother: Nellie Johnson of Elwood, Indiana; her uncles and aunts: Jerry W. Haskell and his wife, Mary Ann of Columbus, Indiana, Larry W. Haskell and his wife, Sharon of Scipio, Indiana; several cousins and other relatives. She was preceded in death by her mother, Shirley Ann Haskell Peddie died, October 7, 2012; her grandparents, Robert Hart, Herbert W. "Hosie" and Geraldine Lauderbaugh Haskell; Paul and Anna Laura Peddie; her uncle, Robert "Bobby" Haskell.

Funeral services will be conducted Monday, March 11, 2013 at 1 p.m., by Bro. Bob Hicks at the Morgan & Nay Funeral Centre, 325 Demaree Drive in Madison, Indiana. Interment will follow in the Grandview Memorial Gardens near Madison, Indiana.

Friends may call Sunday from 5 to 8 p.m. and Monday from 12 Noon until time of the service at the Morgan & Nay Funeral Centre, 325 Demaree Drive in Madison, Indiana.
