FOLK, SUSAN ANN
Ms. Susan Ann Folk age 56, of Starke passed away Thursday, October 4, 2012 in Shands at The University of Florida following a long illness. Ms. Folk was born on August 21, 1956 in West Palm Beach to the late Alton Leroy and Sallie Evelyn Farley. She was a loving daughter, sister, mother, and grandmother, and will forever be in our hearts.
She was preceded in death by her sister Carolyn Smith and her brother Billy Farley. Survivors are: her son: Jackie Bruce Folk and his wife Joanne; sister: Deanna Faison; brothers: Mark Farley and Al Farley; grandchildren: Mason, Trenton, Lloyd and Emmy.
The family will receive friends at the JONES-GALLAGHER FUNERAL HOME on Tuesday, October 9, 2012 beginning at 1:30 PM. Funeral services will follow at 2:00 PM.
