Dr. Philip Paul Czyz, 69, of Port Charlotte, Florida, died May 5, 2012. He was born December 20, 1942, in Warren, Massachusetts, son of Joseph and Nellie Czyz.
Dr. Czyz graduated from the University of Connecticut in 1964 with a B.S. degree. He continued his education at the Pennsylvania College of Optometry, graduating in 1969. Philip proudly served in the Army
as a Captain while completing his clinical optometric training at Martin Army Hospital, Fort Benning, Georgia. He began a solo practice in Port Charlotte in 1973. During his career he received the prestigious Optometric Recognition Award from the American Optometric Association. Philip was an active member of numerous professional societies and service organization including the American Optometric Association, the Association for Research in Vision and Ophthalmology, National Eye Research Foundation, Rotary, and Lions Club. He was a founding investor of the Charlotte Racquet Club and former vision consultant for the Chicago White Sox. His passions included tennis, racquetball, healthy living, and being a proud father to his three children.
He is survived by his loving family, a son Dr. Craig Czyz, and twins Corey and Shannon Czyz who are college students.
Memorial Service will be conducted on Friday, May 11, 2012 at 7:00 P.M. at Charlotte Memorial Funeral Home, 9400 Indian Springs Road, Punta Gorda, 941-639-1171
