Jeffrey Fields Obituary

Dr. Jeffrey David Fields, of Boise, Idaho, passed away on June 10, 2013.
He was born on September 26, 1974 in Muncie, Indiana to parents Cynthia and David Fields. After graduating from Wapahani High School, Jeff attended the Air Force Academy for 3 years and then went on to obtain a Bachelor of Science in Biochemistry at the University of Notre Dame in 1998. He also earned a minor in sculpture art. Jeff received his medical degree from the Indiana University School of Medicine in 2004. He completed both his Family Medicine residency and Sports Medicine Fellowship in Indianapolis, IN.
He has worked with the Indianapolis Colts, the Mixed Martial Arts circuit, and served as team physician for Marian University and several high school sports teams. He provided family and sports medicine care to residents of the Treasure Valley at St. Luke's Capital City Family Medicine.
Jeff and his family moved to Boise in 2010 for their tremendous love of the outdoors. As an accomplished athlete and outdoorsman, Jeff could often be found biking in the Boise foothills, skiing at Bogus Basin, exercising in his garage, and camping in the Idaho wilderness with his family. He also greatly enjoyed traveling and exploring cultures, cuisine, and vistas around the world.
Jeff is survived by his wife, Dr. Helen Jia-Ru Kuo, and daughters Scarlet Yi-An Fields (age 3), Naomi Yi-Jia Fields (age 2), and a baby daughter due in October; parents Cynthia and David Fields; and sister Cortney Coombs.
A local ceremony celebrating Jeff's life will be held in Boise this Saturday, June 15 from 9 to 11 a.m. at Municipal Park, #2 Chestnut Picnic Site, 500 S. Walnut St. Another ceremony will be held in his hometown of Selma, Indiana on Saturday, June 22.
In lieu of flowers, the family requests that any contributions be given to his childrens' college fund. Please send checks payable to Helen Kuo, PO Box 767, Boise, ID 83701

I remember the day Dr. Jeff told, at my last visit with him, he was handing my care off because he was moving to Idaho and my heart broke. He was everything that has been written about him on in this book and more. This shock is even greater, obviously, but at least he was in Idaho and the world still had Dr. Jeff. Wow, I am just so speechless......

I just went to Capitol City Family Medicine today for a Dr. appointment and am truly devastated. He took care of me as late as last March. I sustained a concussion in September 2012 and he worked with me for 6 months. He was a great, smart, very kind Doctor.I am very sad about his passing, we were very lucky to have him here with us. I am truly thankful for the care that he provided me. He was wonderful. My thoughts are with his wife a children.

I had the privilege to have seen him once as a patient. I was struck by the time he took to explain to me in details what I was experiencing and what my options were. He was the first physician I have seen in a long time who not only took the time but trusted me to make the best decision for myself. He was my doctor for 2 years and he was awesome I wonderd why when I went to the st Lukes clinic that I did not get him then I looked his name up online and saw this,I will miss you doctor Feilds.

This made me very sad. I was in his office with my son just the week before he died. He was so thoughtful and helpful as I went through recovery for a concussion, and his insight with sports medicine was invaluable to my son. We had previously been given some "quick fix" answers to some nagging problems, and Dr. Fields talked through a long term exercise plan for how to address and manage the sports related issues we were dealing with. He will be greatly missed. My best wishes for peace and comfort to his wife and family.

Dr. Fields was my doctor since we moved to Boise in 2011. He was very kind and caring and took the time to ask the right questions. He will be missed.

I was a new patient of Dr. Fields. I saw him only twice, but that was enough to know that he was a caring physician. He had a gentle and kind way about him. I felt that I'd been guided in the right direction when he was recommended to me. I'm very sorry to hear of his passing and I send sincere condolences to his family and colleagues.

Jeff was so talented with respect for everyone in all walks of life. His joy for life showed in his work and his adventuresome spirit. Dr. Fields was a resident at IU Family Practice Residency when I worked there too. He also took care of my husband and he did a phenomenal job! We missed him when he left Indianapolis. We will miss his uniqueness even more. Our heart and prayers go out to Mrs. Field and their lovely children.

We were shocked to receive news of Dr. Fields' passing. We chose him as our family doctor shortly after he joined the staff of Capital City Family Medicine. Although we'd been with CCFM for several years, we changed our PcP to him... We found him to be caring, gentle, intelligent and funny- even in the face of fear, sickness and pain that he helped us all through. Dr. Fields was a wonderful physician, and we had the utmost trust in him. Thank you, Dr. Fields, for the care and encouragement you shared with my family. We are so incredibly saddened for his family, his wife and daughters, his parents. You have a man who touched the lives of so many, and made them better with each contact. In those contacts, in his family, he will live on.

Dear Dr. Quo, Though I have not had the pleasure of meeting you, I sincerely hope that one day I may have the privilege of giving you a big hug and letting you know I am truly grieving with you and your little ones. I have always said, and I say it again, that a man, as a man, can not be truly seen, not unless his moral worth and his love worthiness in anything he does, and he lives for, otherwise is seen, and in him, you could see all of that. Jeffrey was my Doctor, an outstanding doctor. I was privileged to work with Jeff as he grew in skill as a physician and leader. As a resident, Chief Resident and Fellow, Jeff's passion for excellence was evident from the beginning and ran parallel to that for his family and all things active involving nature. I have been a patient of Dr. Fields for the past 3 years. He was a phenomenal physician in every way: caring, patient, and calming. His laid back nature and demeanor had a way of always reassuring that things would be okay. I only knew him as my doctor but I wanted his family to know that he made a hugely positive impact in my life. I will miss him and always remember him. I am so sorry for your loss.
