Hazel Pauline Hazelwood Perkins, 79, of Magnolia, passed away Wednesday, Feb. 20, 2013, at the University of Louisville Hospital.
She was a homemaker, a member of Munfordville Separate Baptist Church and a dedicated helpmate to her husband in his ministry.
She was preceded in death by her husband, Rev. John Arthur "Ott" Perkins; her son, Lonzie Perkins; her parents, Robert Harrison "Bob" and Sophia Mae DeVore Hazelwood; two sisters, Ruby Jaggers and Dorothy Dailey and four brothers, Cecil, Otis, Howard and Houston Hazelwood.
She is survived by a daughter, Wanda (Harold) Carter of Greensburg; three sons, Joe Perkins of Magnolia, Russell (JoAnne) Perkins of Hodgenville and James Perkins of Sonora; a brother, Hartsell Hazelwood of Bonnieville; 16 grandchildren; 24 great-grandchildren; two great-great-grandchildren and numerous nieces and nephews.
The funeral is 3 p.m. Sunday, Feb. 24, 2013, at Dixon-Rogers Funeral Home in Magnolia with burial to follow in Union Separate Baptist Church Cemetery.
Visitation is 11 a.m. to 9 p.m. Saturday and after 10 a.m. Sunday at the funeral home.
