Chad Polley Obituary



POLLEY

Chad Wayne Polley, 48, Burlington passed away June 22, 2015 at University Hospital in Cincinnati. He is survived by his wife Melissa (Missy) Polley, his children Nichole and Tyler Polley, mother Glenna (Jerry) Long, all residing in Burlington, sister Kim (Brad) Williams, Williamstown, brother James "Jimmy" (April) Polley, Dry Ridge and his grandchild Kylie. Chad enjoyed Frisbee, hunting and fishing. He loved fireworks, especially the Fourth of July, the ocean and as an artist expressed his talents through tattooing. Chad loved people and the significant events of his life held a special place in his heart. He is preceded in death by his father, grandparents and uncles Johnny, Richard and Bob. A Celebration of Chad's life will be held Sunday June 28, 2015 at the Florence Elks Lodge, 7704 Dixie Highway (lower level) from 1-4 PM with a closing prayer at 4 PM.
