PEORIA - Samuel Haueisen, DDS, RPH, 57, of Fort Worth, Texas, formerly of Peoria, Ill., died Tuesday, Dec. 21, 2010.
Funeral services will be at 2 p.m. Monday, Dec. 27, at Thompson's Harveson and Cole Funeral Home in Fort Worth, Texas. Visitation will be one hour prior to service time. Burial will be in Greenwood Memorial Park in Fort Worth, Texas.
In lieu of flowers, please honor Sam's memory with a gift to Casa Manana's Children's Conservatory (CPAC), which is their school for youth, 3101 W. Lancaster Ave., Fort Worth, TX 76107.
Samuel was born Aug. 24, 1953, in Peoria, Ill., the only child of Carroll and Georgia Haueisen. He married Dianne Sue Brown on Dec. 28, 1974, in Peoria, Ill.
He earned his RPH degree in 1976 from Drake University in Des Moines, Iowa, and his DDS in 1986 from Baylor College of Dentistry in Dallas.
He established Joshua Family Dentistry in 1986 and practiced there for 24 years until his death.
Sam took a deep interest in his children's passions, putting his children and their activities first; Brice in music and magic, Emily in film and music and Trevor in acting and music. His children's friends considered him their second dad - entertaining them with stories and jokes.
He loved the Beach Boys, Beatles and Willie Nelson.
Survivors include his wife of 36 years, Dianne; children, Brice, Emily and Trevor Haueisen; and aunt, Jeanette Herink.
