David Cook Obituary
Dave is survived by Lisa Cook and his children, Ryan and Emma; his parents, Harold and Maryann; his sisters, Cheryl Hite (and JB) and Kathleen Cook; and his niece, Kate.
Celebration of Dave's life will be held on Saturday, April 22, 2017 at 2pm at Empire Canyon Lodge at Deer Valley Resort, the address is: 9200 Marsac Ave, Park City, UT 84060. Guestbook to post messages and tributes for the family is available at: www.HolbrookMortuary.com
Donations can be made to the National Center on Addiction and Substance Abuse (www.centeronaddiction.org).
A fund has been set up to support Emma and Ryan through the Wells Fargo bank in Park City.
