MAPLEWOOD — Charlotte "Ann" Smith, 82, of Maplewood, passed away Tuesday, Oct. 6, 2015, at 9:35 a.m. at Heritage Manor Nursing Home in Minster.

She was born on Dec. 4, 1932, in Port Jefferson, Ohio, the daughter of the late Howard and Luella (Shaffer) Rike.

In 1954, Ann married Thomas F. Smith, who survives along with 11 children, Thomas (Connie) Smith, Tony (Karen) Smith, Margaret Bowman, David Smith, Kathy (Don) Young, Pat (Leslie) Smith, John (Bonnie) Smith, Jeff (Robyn) Smith, Ben (Jo) Smith, Ted (Christal) Smith and Elizabeth Morris; two sisters, Nancy Leach and Judy Duckro; 20 grandchildren; seven great-grandchildren; and six great-great-grandchildren.

She was preceded in death by one son, Stephen Smith, and two sisters, Mary Applegate and Carol Samuels.

Mrs. Smith was a 1950 graduate of Sidney High School. She was a homemaker and a long-time member of North Broadway Church of Christ.

Ann was a loving wife, mother and grandmother who sacrificed everything for her family. She had a passion for family history and was a member of the Shelby County Genealogical Society.

Ann's family would like to thank the nurses and staff at Heritage Manor for the loving and compassionate care given to their mother.

Funeral services will be held Monday, Oct. 12, 2015, at 10:30 a.m. at Cromes Funeral Home, 302 S. Main Ave., Sidney, with the Rev. Brent Wright officiating. Burial will follow at Glen Cemetery in Port Jefferson.

The family will receive friends on Sunday from 1 to 4 p.m. at the funeral home.

Memorials may be made to North Broadway Church of Christ in Ann's memory.
