HENDERSON, NANCY JANE  
NANCY JANE HENDERSON, 72 Cypress Lanes Bowling Center LAKELAND - Ms. Nancy Jane Henderson, age 72, died of heart failure Tuesday, January 27, 2015, at Southland Suites, Lakeland. Born in Waterbury, CT, on February 18, 1942, she came to Lakeland from Newport, MA, in 1962. She was a league organizer at Cypress Lanes Bowling Center in Winter Haven. She was a member of Lamb of God Lutheran Church in Haines City. Ms. Henderson is survived by her daughters Lisa Rae Knoles, Eagle Lake, Karen Kay Tofanelli, Diana Lynn Gilley, both of Winter Haven; five grandchildren; one great great grandchild. Visitation will be from 5 to 8 p.m. Saturday, Jan. 31, 2015, at Crisp-Coon Funeral Home. Graveside services will be at 2 p.m. Sunday, Feb. 1, 2015, at Rolling Hills Cemetery, Winter Haven. In lieu of flowers, donations may be made to the Polk Co. Humane Society or the Alzheimer's Association.

HORNER, JACK JAMES  
JACK JAMES HORNER, 51 Truck Driver LAKELAND - Jack James Horner, age 51, passed away at his home surrounded by family on the night of January 25, 2015. He lost his fight to brain cancer. He was born in Lakeland, FL, on May 27, 1963. He graduated from Kathleen Senior High School, class of 1981, and married his high school sweetheart in 1982. Jack worked as a truck driver and served in the United States Air Force from 1988-1991. He was a drummer who loved music. He also enjoyed spending time at Disney with his family and also running the Disney marathon races. He was a Christian man who deeply loved his family and friends. He always had a wonderful sense of humor! He was preceded in death by his father William Horner and his sister Holly Horner. He is survived by his mother Dorothy Horner, wife Janice Gay Horner, son Joshua Horner (Tiffany Horner), daughter Pamela Horner, sister Karen Horner (Dave Ansel), brother William Horner (Karen Herr-mann), sister Dori Kochenburger. He also had a niece Nicole Kochenburger, and nephew Neil Kochenburger. Jack also had a granddaughter Mackenzie Rae Horner, born in 2013. A memorial service will be held on January 31, 2015, at 11:00am at Faith Assembly of God located at 4240 North Frontage Rd., Plant City, FL, 33565. A gathering celebrating the life of Jack Horner will be held at the social hall on site.

HUMPHREY, JAMES W.  
JAMES W. HUMPHREY, 96 WINTER HAVEN - James W. Humphrey, 96, was born in Sykesville, PA, November 20, 1918, and died in his home in Winter Haven, FL, December 15, 2014. He is preceded in death by his wife of 72 years, Evelyn, and is survived by daughters Peggy Beck and Patty Muller, 2 grandchildren and 2 great grandchildren. Jim and his wife, Evelyn, graduated from Sykesville High School as classmates in 1935 and married in 1941. At age 16, Jim attended the Williamson Free school of Mechanical Trades for young Pennsylvania boys. It was there that he learned to excel in woodworking. From 1944-1946, Jim served in the Navy in the Pacific on a ship repair unit. He entered Japan after the war. Following the war, he attended Penn State, graduating in 1950 with a degree in mechanical engineering. He and his family lived near Erie, PA, until moving to Florida in 1955. Here he took a job with George L. Simmonds Co. as an engineer. Jim was active in Girl Scouting, serving on the Camping Committee that bought land and created Camp Wildwood. He served on numerous committees at his church, Beymer Memorial United Methodist. In 1973 he and Evelyn became active with Airstream Caravanning, leading groups throughout the U.S. and Canada. Jim Humphrey is known as a talented and generous man. His memorial service will be held in the sanctuary of Beymer Memorial United Methodist Church, 700 N. Lake Howard Dr., Winter Haven, on Saturday, Jan. 31, 2015. A memorial organ recital begins at 9:30 followed by the service. WILLIAM G. 'JERRY' JOHNSON, 80 Lineman Supervisor LAKELAND - William G. 'Jerry' Johnson, 80, passed away peacefully on Wednesday, January 21, 2015, from cancer. He was born in Reform, AL, on May 21, 1934. Jerry came to Lakeland 26 years ago after retiring from Florida Power & Light as Lineman Supervisor with 32 years in Miami, FL, and Arcadia, FL. Jerry, also, served our country in the Navy. He was preceded in death by his wife, Ida Johnson of Lakeland, FL. He is survived by his daughter, Donna Klerk, Henderson, NV; sons, Brad Johnson, Miami, FL; Greg Glenn, Nashville, TN; and Gregg Johnson, Deltona, FL; sister, Carolyn Bell, Dallas, TX; 8 grandchildren; 2 great grandchildren. MELNYK, EDWARD  
EDWARD MELNYK, 79 ST. AUGUSTINE - Edward Melnyk, age 79, of St. Augustine, and formerly of Lakeland, went home to be with the Lord on January 20, 2015, at Brookdale Assisted Living Facility in St. Augustine, surrounded by his wife and family. Ed was born and raised in Elk Point, Alberta, Canada. After completing his high school studies Ed married his bride and sweetheart, Beatrice, then continued his secondary studies at the University of Alberta where he received a Bachelor of Science in Mining Engineering. Upon graduation, and motivated by his adventurous spirit he began his mining career high in the Andes Mountains in Cerro de Pasco, Peru South America, where he mined copper deep underground. In 1967, he relocated to Tucson, Arizona, to broaden his career experience mining copper in an open-pit with Anaconda Copper Company. In 1974, Edward relocated to Lakeland, Florida, where he completed his career strip-mining phosphate with International Mineral and Chemical Corporation (IMC) where he retired after attaining the position of Florida Operation's Production Manager. In 2012, he and his wife, Beatrice, moved to St. Augustine, where they have resided for the past 3 years. Throughout his adventurous life, Edward not only loved his family and job, but thoroughly enjoyed traveling, covering the entire United States by car. He and Beatrice also traveled extensively to Europe, the Caribbean, Hawaii and Canada. Edward is best remembered for loving Jesus, his Savior, and he was a longtime member of Covenant Presbyterian Church in Lakeland, Florida. In his spare time, his favorite hobbies were hunting deer, turkey and hogs, as well as deep sea fishing and golfing. Above all, Edward will be remembered as a devoted husband, father and grandfather, and he will be deeply missed. A celebration of his life will take place 11:00 AM Saturday, January 31, 2015, at St. Johns Family Funeral Home. Flowers are gratefully declined and those wishing may make a contribution in his memory to AmVets or to the Wounded Warrior Project. He is survived by his wife of 57 years Beatrice I. Melnyk of St. Augustine; a daughter Diana Justice, and her husband, Keith, of St. Augustine; sons David E. Melnyk, and his wife, Marlowe of Fellsmere, FL, and Frank J. Melnyk, and his wife, Suzanne of Mulberry, FL; a sister Mary MacIntosh of Alberta, Canada; brothers, Onufer Melnyk, Gerald Melnyk, Tommy Melnyk and Boris Melnyk, all of Alberta, Canada; and seven grandchildren. St. Johns Family Funeral Home is in charge of arrangements.

MITCHELL, PHILLIP WAYNE "MITCH"  
PHILLIP WAYNE 'MITCH' MITCHELL, 72 DAVENPORT - Phillip Wayne 'Mitch' Mitchell, 72, of Davenport, FL, joined an endless procession of saints on Tuesday, January 20, 2015, which includes his beloved parents, aunts and a granddaughter, Sabrina. Born on December 1, 1942, in Philadelphia, PA, to Pearline Giles and Raymond Mitchell. His loving aunts Beck and Inez raised him in Laurel, DE. 'Mitch' attended Orange Corner and Paul Lawrence Dunbar schools and graduated from William Jason High in Georgetown in 1960, then enlisted in the U.S. Air Force. After 20 years of and 2 tours of duty, he retired in 1980 as a Sr. Master Sgt. He continued to serve and support military veterans by becoming a Commander of the American Legion Post #509 and a member of the VFW, both in N.J. He began his 2nd career working for NJ State Dept. of Corrections as an Officer, then retired in 2001. Phillip had a wonderful, active and full life and was always surrounded by fun-loving folks. After retirement, he and his lovely wife Paulina 'RV'ed' for several years on a full-time basis. He belonged to a number of social organizations: 33rd Degree Mason, an Officer in the Esquire Motorcycle and Van Club, Treasurer of the 'Group Camping Club' and member of the National African American Recreational Vehicle Association where he traveled around the U.S. Eventually they moved to Davenport, FL, where he became a member of the AMVETS. He leaves behind several loved ones to cherish his memory: a loving wife: Adeline Paulina Mitchell, 6 children: Derrick (Doris) and Ericka (Marcus) Fort Worth, TX, Gwendolyn of Aledo, TX, Michael (Cherese) Mansfiled, TX, Lief (Sanders) Oxon Hill, MD, and Michelle of Bowie, MD, 19 grandchildren: Latisha, Kasandra, Ebony, Tomika, Kinnina, Michael P, LaMesha, Scott, Natalia, Quincy, Naquirra, Phillip, Quanel, Joshua, Nielah, Jada, Mehki and Malachi, five great grandchildren: Tomir, Kason, Airyanna, Tamia and Siyani, six siblings: Clifton (Lealar), Marvin (JaVette), Patricia (Bishop Johnnie), Mark, Robert (Cherise) and Cheryl (Wallace), a sister-in-law: Harriett Banks, and a host of nieces, nephews, cousins, extended family members and friends. 'Mitch' will repose at Claude Holmes Sr. Memorial Chapel on Friday, the 30th day of Jan. from 5-7 pm. The homegoing service will be held on Saturday, the 31st at 1100 hours at the New Mt. Zion MB Church, 713 N. 8th St., Haines City, FL, with burial at the Florida National Cemetery and Military Honors on Monday, the 2nd day of February at 1100 hours. JOSHUA ALLAN MUDGETT, 38 MULBERRY - Joshua Allan Mudgett, age 38, passed away January 24, 2015, in his sleep. Joshua was born May 17, 1976, in Syracuse, New York. Joshua has lived most of his life in Mulberry. He worked in construction. He was a devoted father to his daughter Maddie. Joshua is survived by his daughter Maddie Mudgett, mother and step-dad Janette & Randy Starling, father Allan Mudgett, brothers Ryan & Doug Nicks and sister Maureen Nicks.
