A Funeral Service will be held for the late Miss ANNA LISA CHMIEL at 2.00 pm on MONDAY (October 8, 2012) at the Wattle Park Chalet, accessed via Monsbourgh Drive, off 1012 Riversdale Rd, Surrey Hills. Family and friends of Anna are warmly invited to attend a celebration of her life. The Service will be followed by a time to reflect and reminisce.

In lieu of flowers, the
family welcomes donations
to an Anna Chmiel Memorial Fund, being established to provide scholarships and other financial assistance to
music students through
Melbourne Youth Music
and Monash University
School of Music.

Following a private
Cremation, Annas ashes
will be interred in her maternal grandparents grave site at the
Templestowe Cemetery (P59).
