FRED MD MSc FRCSC FACS FAANS Professor, Department of Surgery, University of Toronto The Crean Hotson Chair in Skull Base Surgery The Alan and Susan Hudson Chair in Neuro-Oncology It is with profound sadness that the family of Dr. Fred Gentili announce his peaceful passing on Saturday, January 15, 2022 in Toronto, surrounded by his wife, sons, and mother-in-law. Born to Angelo and Lina Gentili in Sarnano, Macerata, Italy, he will be lovingly remembered by his wife, Gina; children: Michael (Bob) and David (Devon); grandchildren: Sebastian, Oscar, Alexander, and Samuel; brother: Albert; as well as numerous relatives and friends. He impacted countless lives with a tireless dedication to his patients, a passion for innovation, and mentorship of the next generation of neurosurgeons. Outside of the operating theatre, Fred never shied away from a lively debate over good food and fine wine, often hosting family, friends, and visiting fellows. Fred enjoyed travel, whether to attend conferences to further the field of neurosurgery, or to visit loved ones on family vacations in and around Sarnano. A graduate of the University of Toronto (UofT) medical school, he was awarded a McLaughlin Fellowship to pursue advanced training in skull base and micro-neurosurgery, completing clinical fellowships at the University of Zurich with Prof. G. The family wishes to express their gratitude to the Toronto Western Hospital, UHN Radiology, and PMH Radiation & Oncology staff for their expert care and treatment. In lieu of flowers, donations to the Dr. Fred Gentili Skull Base Clinic Fund at UHN Foundation would be appreciated by the family. Please visit UHNfoundation.ca/Gentili or call 416-603-5300. Extending my deepest condolences to Dr. Gentili's family, many friends and colleagues. A great loss to the medical surgical community.
After many surgical hours he made sure to visit me in recovery after a very long day with a big smile on his face. He was an outstanding surgeon and asset to UHN and to all of us lucky enough to be touched by his skill. RIP great man.

In 2008, when I was trying to persuade Dr Gentili to consider doing a very risky brain surgery on a tumour that had lodged itself in a precarious part of my brain, the thought that I might be alive all these years later to write this (because he saved my life), this breaks my heart to think about.
I cannot imagine how difficult this must be for those who loved Dr Gentili.
I send sincere condolences and deep gratitude.
I think of those he mentored, worked with and knew him well and send sweetness to each of them too.
Our world lost a someone who really made a difference.
Thank you.
After surgery, I went through weeks of radiation treatments. I am including a self portrait of me during that process.

Dear Gina and Sons,
Your loved one was very special to me in so many ways, and I can only imagine the depth of your loss.
He was a wonderful colleague to me for the past 25 years or so that we have worked together. He was outstanding in his field and outstanding as a person. He was very kind, very approachable and very generous to his colleagues and patients. As a neurosurgeon and colleague he was the best, and so it it must be an incredible loss to you, but hopefully his good deeds and generosity will have paved the way somehow for your future filled with those good thoughts and the inspiration they generate.
With great respect for Fred Gentili and his dearest,
Charles Tator

I just stumbled across this and was sad to read this news. Dr Gentili treated my mom seven years ago at Toronto Western and we will forever be grateful. A huge thanks to him and to his family who I am sure sacrificed time with him so he could care for others. We are truly indebted. I remember how all the nurses would stand at attention when he entered my mother´s room in the ICU. He no doubt commanded a lot of respect. Sincere condolences to his family.

My Deepest Condolences to Dr Fred Gentili and his Family. Dr Gentili Saved my Life back in 2097, I am so grateful to him that I am still here today to see my daughter Emily grow up. Unfortunately because of COVID I have not been able to see Dr Gentili in 3 years, but like previous patients I never got to thank to him for saving my life, so thank you Dr Gentili For saving my life and allowing me to continue on to be a husband to my wife Antonietta and a father to my daughter Emily.

I wrote this to Fred when I first heard he was ill but thought Gina and the family might like to read it. My deepest sympathies to all of you for your great loss. His teaching, his enthusiasm for his surgical craft, and his kindness live on brightly in us, those were so privileged to be the recipients of his mentorship.

Dear Fred,
I was really shocked to hear about your illness. There is a great deal of concern and support for you from your extended neurosurgical family. I thought I would take this opportunity to tell you how much you mean to me.
You are one of my neurosurgical heroes. Even though we haven´t worked together for a long time, I often think about the lessons I have learned from you with respect to microneurosurgery, particularly in the middle of my cases, while I´m under the microscope. Intracranial microsurgery is my absolute love, and I think I got that from working with you at Toronto Western in 1993. I have strived to emulate your surgical ambition, to be able to do something that maybe others cannot, to achieve a complete surgical removal, but temporizing with what is realistic, and not to go too far because then you can be foolish. You will never be a great neurosurgeon unless you push that envelope, as long as you know the limit. Master surgical technical skill still counts for so much. Despite all the exciting advances in biology, surgery remains critical for so many neurosurgical conditions. Our future needs of course to look to biology but surgery is still an anchor. Our patients will have the best outcomes if they start with the best surgery. The techniques you employ and the approaches you use stick with me, your techniques for arachnoidal dissection, your use of sucker and bipolar, and your delicate handling of critical structures are techniques that I learned from you and that I use with every microsurgical case. Your attention to the maintenance of the highest technical skill through practice resonates so deeply with me. One of the other big things is your patience and stamina. To get a good result you have to be extraordinarily persistent and patient. Follow the case through and don´t give up. You have been such a fantastic surgical teacher to so many.
Above all, you also care deeply about people and their outcomes, some other master surgeons don´t always give that impression, but you always do.
You are always gracious to others, and are an exemplary professional and colleague. Always willing to take on what others cannot. Always willing to come to SickKids and help us do things we cannot. I see you as the heart and soul of the UHN neurosurgery service and our U of T Division.
I hear your voice in all of my surgeries and for that I am forever grateful to you.
I will be thinking about you and your family in this challenging time.
Kind regards,
Peter

It is with great sadness to hear of the passing of Dr. Gentile. Over 25 years ago my husband was operated on by Dr. Gentile and every year since, he has gone for MRI/TWH follow-up appointments. It was always a pleasure to meet up him on these annual visits. He always made my husband (and myself) feel comfortable. He was a fine doctor who will be greatly missed by all. Our condolences to the family.

I am so sorry for your loss. I am another of his very grateful patients. Many years ago I arrived at TWH emergency triage with an urgent note about my situation from another doctor. Fortunately, Dr. Gentili was on call overnight. He did the surgery at 3 am and greeted me with a smile at 6 am.
