LeAnn Denise Crooks

LeAnn Denise Crooks, 53, Springfield, Missouri departed this life October 8, 2016 at Cox Medical Center South.

She was born October 2, 1963 at Berea, Ohio to William and Doris (Roche) Crooks.

LeAnn was diagnosed with CDLS nearly six months after birth and was among the older survivors of CDLS.

She was preceded in death by her mother Doris Crooks.

Immediate survivors include her father William Crooks and one sister, Janet Crooks, all of Springfield, Missouri, her grandmother, Frances Crooks, two uncles, Joe and David Crooks, cousins, Matt, Katie and Virginia, all of the state of North Carolina, nieces and nephews, Sarah, Danny, Nicole, and Brandi. Special Thanks to LeAnn's care staff, Cindy, Holly and Magda.

Visitation will be held from 6:00-8:00 PM Wednesday, October 12, 2016 at Greenlawn Funeral Home East with funeral services at 2:00 PM Thursday, October 13, 2016 at Greenlawn Funeral Home East.

Burial will follow in Greenlawn Memorial Gardens.

In lieu of flowers, memorial contributions may be made to the CDLS Foundation in care of the funeral home.
