Dr. James Leslie "Dr. Jim" Forsberg, 56, of Libertytown, died Wednesday, June 11, 2014.

Born Aug. 18, 1957, in Mineola, New York, he was the son of Phoebe E. Forsberg, of Vestal, New York, and the late Leslie I. Forsberg. He was the husband of Ellen Margaret Stehle Forsberg, his wife of 35 years.

He was raised on Long Island, New York. He received a bachelor of science in biology degree from Hofstra University, in Hempstead, New York, in 1979.

Following his marriage, they moved to Rochester, New York, where he attended the University of Rochester Medical School, graduating in 1983. After completing a family practice residency program at York Hospital in York, Pennsylvania, he joined three friends, Drs. Stephen Sikorski, Paul Forst and Michael McEvoy, as founding partners of Carroll Primary Care, with offices in Westminster and Sykesville. Subsequently, the Sykesville office has been relocated to Eldersburg.

He had been serving as a board member of the Alpha Crisis Pregnancy Center of Reisterstown, and as an elder at Fredericktowne Baptist Church in Walkersville  for the past several years.

Surviving, in addition to his mother and wife, are children Emily, Luke and Abigail Forsberg, all of Libertytown; brothers and sisters-in-law Peter A. and Karen Forsberg, John R. and Joyce Forsberg, Thomas G. and Janice Forsberg, Philip D. and Ellen Anne Forsberg, and Timothy D. and Debby Forsberg; sister and brother-in-law Elizabeth and Mark Watts; a mother-in-law, Mildred A. Stehle; brother-in-law Bill Stehle and wife Teresa; and numerous nieces, nephews, grandnieces and grandnephews.

A celebration of life will be held at 1 p.m. Saturday at Fredericktowne Baptist Church, 8645 Biggs Ford Road, Walkersville.
