Evangelista, Teofilo L. "Phil" Dr. Teofilo L. Evangelista, 93. Former Chief of Anesthesiology at Appleton Medical Center DELAFIELD, WI - Dr. Teofilo L. Evangelista, 93, died peacefully on June 17, 2017 surrounded by family. He was the loving and devoted husband to Frances Evangelista of Delafield, WI. Dr. Evangelista was born in Navotas, Philippines to Luciano and Estefania Evangelista in 1924. After earning his medical degree at Santo Tomas University in 1952, he immigrated to the United States in 1955. Dr. Evangelista, or "Phil" as he was known to his family and friends, interned at Deaconess Hospital in Milwaukee, WI., where he met and married his beloved Frances. They were married on June 17, 1955 and the two moved to Jamestown, North Dakota where he completed his residency in general medicine in 1963. Phil went on to complete a fellowship in Anesthesiology at the University of Minnesota in 1965, and then he moved to Appleton, WI to become a partner at Fox Valley Anesthesiologists. He and Frances lived in the Appleton area for many years, where they raised their family. In addition to private practice, Phil ultimately became Chief of Anesthesiology at Appleton Medical Center. Phil enjoyed gardening, nature, good wine and food, and entertaining. Family and friends will never forget his generosity and his gentle, patient, and very welcoming nature. We will remember the many family parties where everyone was welcome and greeted with an offering of wine. Most didn't know he was also an accomplished painter and could have made that a successful career path as well. He will be well missed by all who knew him. His grandchildren will greatly miss their "Nonnu." In addition to his wife, Phil is survived by a son, Mario Evangelista and his wife Joanne Coyle of Marstons Mills, MA, his daughter Richelle Peddie and her husband Jon of Delafield, WI, and a stepson, Eugene Zappaterreno of Williston, North Dakota. He also leaves behind six beloved grandchildren, Sarah and Anna Evangelista; Brooke and Jackie Peddie; and Jaimi Davis and Christina Fairbrother. He is also survived by his brother-in-Law John Tarantino and his wife Joan; his sister-in-law Angie Schlosser and her husband Joseph, as well as many great-grand children, nieces and nephews. Visiting hours will be held on Thursday, June 22, between 10 a.m. and 11 a.m. at Good Shepherd Catholic Church in Menomonee Falls. A Mass of Christian burial will follow at the Church at 11 a.m. Interment will follow at Holy Cross Cemetery in Milwaukee. The family wishes to thank Angel's Grace Hospice for their exceptional care and kindness to Phil during his time of need.
