Jeffrey Warren "Jeff" Holtzclaw, 33, Radcliff, formerly of Lebanon, passed away Tuesday, Nov. 13, 2018 in Hodgenville. 
He was born on Nov. 24, 1984 in Campbellsville.
He was preceded in death by: his maternal grandfather, Lambert Livers; paternal grandparents, Albert and Jewell Holtzclaw; uncle, Kenny Livers; and partner, Joseph Bell.
Survivors include: daughter, Elizabeth Dawn Holtzclaw; son, Lucas Warren Holtzclaw; mother, Margie Livers Holtzclaw of Lebanon; father, Joey Holtzclaw of Campbellsville; brother, Elijah Livers of Lebanon; two sisters, Alexis Livers and Olivia Livers, both of Lebanon; maternal grandmother, Agnes Deverial Livers of Lebanon; three aunts, Theresa (Keith) Richerson, Cheryl (Terry) Yates, and Jackie Livers; four uncles, Joe Livers, Walter (Lydia) Livers, Jimmy Livers, and Chris Livers; and paternal uncle, Mark (Sherri) Holtzclaw.
Funeral services were held at 1 p.m. Saturday Nov. 17, at Open Arms Community Church with interment in Our Lady of the Hills Church Cemetery.
Pallbearers were Clay Benningfield, Joey Benningfield, Walt Livers, Johnathon Fulks, Mike Yates, Charlie Richerson, Kevin McCandless, and Jimmy Dan Livers.
Honorary pallbearers were Matt Livers, Justin Beavers, Dale Porter, Marshall Paul Shuck, Andy Richerson, Lynn Yates, Elijah Livers, and Bobby Grayson.
