Dear Family Members of Dr Herbert Forman,
I have been searching for news of Dr Forman since the internet became popular. I was also a patient of Dr Forman's, but I became his patient in 1985 at Michael Reese's Psychiatric & Psychosomatic Institute. I was referred to him, by Dr Bey, from Brokaw-Mennonite hospital in Normal, IL. I had been transferred to Chicago from the hospital there, because I was a college student. At the time, most of Dr Forman's patients were geriatric.
I'm very grateful to Dr Forman for being my doctor and for really listening to me.
I would like his wife and children to know that I went back to school and became an RN. I graduated at the top of my class in December of 1999. I worked for 20 years as an RN. I wanted to thank him for saving my life when I was17-20 so I could have mental health recovery.
There was a day where, I didn't think I could take the pain of living, even one more day. But I kept living and have lived for 39 more years.
Thank you Dr Herbert Forman.

While I did not know Herb very long, he was a kind, warm and loving man and I will miss him. Herb was part of the same support group I belong to at the Cancer Wellness Center in Northbrook, IL

I will always be grateful for Dr. Forman's dedication, warmth, neverending support and kindness. He was not only my doctor, he was also my guardian angel. Always making sure that I got the care I needed. He continually went that extra mile for me, never giving up or passing judgment. There will always be a special place in my heart for Dr. Forman. Without his care I would not be where I am today-Healthy and happy.

I did not work directly with Dr. Forman at Kenneth Young Center, but everyday I hear from different staff members what a wonderful and caring doctor he was to all his clients. My thoughts and prayers go out to his family.

As a "newer" employee to the Kenneth Young Center, I knew Dr. Forman only a short time. However, his warmth, compassion, and humor were evident in every conversation we had. He will be missed greatly by staff and clients alike. My sincerest condolences to his family during this difficult time.

I have had the pleasure of working with Herb for the past 10 years. His compassion, kindness and caring level toward his patients was of the highest standard. He was a rare person with a big heart, and I will remember him with fondness. My deepest sympathy to his family.

We grew up together on Millbrook Lane......it seems like so long ago. Herbie was my brother's best freind. We shared the pleasures of a marvelous childhood: shooting baskets, croquet, running bases, statue maker, exploring the construction sites, cops 'n robbers, hide and go seek, you name it...........and of course our beloved Cubs. I have such great memories. Please accept my sincere condolences and special love to Mrs. Forman and Amy.

I knew Herb on the management team here at Kenneth Young Center. I am not a clinician, but I saw the depth of love and respect for this man, from clients and clinical staff alike. He was a caring, gentle, wise and thoughtful man, a capable and thorough doctor. His humor was quiet too, which made it all the more effective. We are sad and will continue to miss Dr. Forman. Dr. Forman has impacted my life for the better. He will NEVER be forgotten by me and for all he has done to help my life. He is the most wonderful doctor I have ever encountered and he will never leave my memory. I wish his family my sincerest condolences as he will be truly missed.
