Jay Crooks Obituary
1940-2015
Jay Martin Crooks, 75 of Pensacola, FL passed away peacefully at the Specialty Health and Rehab Center, Wednesday, October 14, 2015 after suffering a medical setback due to Alzheimer's. Jay was born in Clearfield County, PA and is the son of the late Wayne and Alice Crooks. Jay proudly served 20 years in the U.S. Navy until his retirement and settling in Pensacola for the past 32 years. He also worked for the Dept. of Juvenile Justice for 10 years before he retired. In his earlier years, he was an avid scuba diver and golfer. Jay loved being around family and friends, music and good food.
He is preceded in death by his late wife, Sally. He is survived by his children, Kelly (Amos) Kardos and David (Teresa) Crooks; grandchildren, Justin Mongoso, Jamie (Brian) Erlwein and Austin Kardos and one great-grandchild, Erik Erlwein.
Visitation will be held Monday, October 19, 2015 at Faith Chapel Funeral Home South from 10:30 a.m. with the funeral to begin at 11:30 a.m. Interment will follow at Barrancas National Cemetery.
In lieu of flowers, the family requests memorials be made to the Alzheimer's Association,www.m.alz.org in memory of Jay M. Crooks.

so deeply saddened by this....i was your dad's secretary at HRS when he was the volunteer coordinator...my name at that time was wanda teel....one of the most sincere and sweet man i have ever known....RIP Jay...
