Funeral service and committal to be held at Preston Crematorium on Monday, January 10, 2011 at
3.15 pm Family flowers only please.

Donations if so desired payable to Lancashire Teaching Hospital NHS Foundation Trust,

support the dedicated work of the
Acute Pain management team.

A short memorial Mass to celebrate the life of Philip and to give thanks for the dedicated work of Hospital staff will be said in the Royal Preston
