Beloved daughter of Elsie Caulkins; loving wife of the late John "Jack" Forrest; dear mother of Annette Howell, Steven (Diane), Linda (Dennis) Nilson, Mary Hopkins, Mark (Randi), Maureen (Dan) Fisher, Scott (Nancy), Michael (Cindy) and Kevin. Proud grandmother of 22, she was a great grandmother and a great great grandmother. Her life will be celebrated at Mass on Friday, January 15, 2021, at 10:30 am, at St. John Cantius Church, 825 N. I'm so sorry to hear about Donna. She was my boss for three years in the Copy Center at AIC. She was generous, thoughtful, funny, and sassy. I am lucky to have spent time with her all those years ago. I will remember her fondly!!
