Brandon Michael Polaski born October 16, 1976 passed away October 15, 2018. He is survived by his parents Don Michael and Vicki Polaski, children Alyssa, Aiden and Addison; special aunts and uncles Rick (Debbie) Boehm and John (Jean) Akers, cousins Tommy Kidd, Justin Boehm and Corey (Nikki) Akers; longtime girlfriend Erin Casto, Private family services will be observed. In lieu of flowers please make donations to The American Heart Association in his memory.
