JOHN A., M.D., of Phila., on Sept. 29, 2011 at the age of 88. Beloved husband of Nancy (nee Hare); dear father of Nancy G. (the late Edward) Murphy, Karen H Koltes (John) Tschirky, John A. Koltes III (Patricia), Steven F. Koltes (Corinne); survived by 7 grandchildren and a sister Eleanor Barwis (William). Relatives and friends are invited to his Memorial Service Monday (Oct. 3) 9:30 A.M. at The Presbyterian Church of Chestnut Hill, 8855 Germantown
