My memories of Jack's kindness wil always remain in my heart. He was a gentleman who sincerely cared for others. He was a true humanitarian who's legacy is his beautiful family and an enduring servitude for his community.

Jacque and Family...
My love and prayers go out to all of you. For the short time I knew Jack, he truely touched my heart. I have never known a kinder more gentle guy, always there with a smile and a hug. Heaven will be a even better place with him there.

Growing up in the 1960's, the Sullivans and my family lived across the street from one another. What a wonderful, wonderful man. He not only delivered my three beautiful children in the 1970’s, but went so far as in the late 60's to come over one night to help our cat, of all things, who was having a difficult delivery. Yes, he also brought five kitties into the world. He will truly be missed and always remembered.

My sincere condolences to the Sullivan Family on the loss of husband, Dad and Grandpa. I remember Jack always as a nice fellow. Last time I saw him was at the Chaffey Class of '53 Reunion.

I will miss sharing with Jack as we did so often over the years on many nights especially on Friday evenings. He was a wonderful friend and mentor. He gave so much to all of us.

It breaks our hearts to see a wonderful man pass on from us, but we know that Heaven will surely welcome him with open arms. Dr. John as we called him, always had great bedside manner. He was truly interested in your life no matter how busy he was. Dr. Walter Sullivan was my grandparents Doctor, as well as my parents and mine. Dr. Walter delivered my Father and me. Dr. John Sullivan delivered both of my children and was our family doctor for many years. Wow! I know several people that had him as their doctor and the consensus is unanimous—He was a great doctor.

I am so sorry to hear of Dr. John's passing. He was always ready with a smile and gave excellent care to our family. My kids will never forget "the shot docs", along with Norma, Judy and of course, the famous Tootsie Pops that were the highlight of every visit.
I will miss seeing the "Dynamic Duo" rounding the halls of the hospital......my thoughts and prayers are with you all.
They just don't make Dr's like the Sullivans any more........

Jacque, Karen, Bridgett and Teri and (sons)all the Sullivan family, my thoughts and prayers to all of you. I've only known Jack for a few years but he truly left an impression. He never let this illness keep him down. He continued to work in the yard, take drives to Home Depot:) and have his favor dessert just a few days before the Lord took him home. He truly loved his family, friends and life. He was a real angel to all on earth. But it was his time to be with the "Big Dude", and rest now. He was loved by many and will be missed by ALL. God Bless you Jack.....I feel blessed just knowing you. My thoughts and prayers to all the family during this time of loss.

The memories left behind from Dr. Jack are endless - not only did his father Dr. Walter, care for our family, Dad, Mother, me, through a devastating auto accident, my sister Diana (whom he still was taking care of until recently) our children, Marty, Steve, Erin and Chris and was always so caring, loving and just "there for us"! My husband Martin and I graduated from Chaffey High School in 1953 with him and have been so fortunate to see and be with him at all of our Class Reunions, always so happy and fun to see him and Jaclyn! We will miss his sweet and funny ways and will have him in our hearts always!

To the family of Jack Sullivan, I am deeply saddened to hear of his passing. I graduated with Jack from Chaffey High School in 1953 and remember him well. My condolences to the Sullivan family. Larry and I were patients for many years and when Larry would be with me, or see Dr. John by himself, they would always get into stories about WWII which Larry was in or about VN. He had that special gift of really caring for his patients.

Jacque and Family,
Dr. John was the doctor for my mother and father, myself, my wife and son. He was a dedicated life long friend to my parents and was instrumental in solving and guiding them in health related matters. More importantly, he was a great humanitarian who put others first. I cherished the interest he showed in me outside of health issues and amazed at the way he could remember what we talked about the last time I saw him, eventhough it had been a length of time. Jack was an inspiration to me. I've watched him for the past fourteen years giving everyone in his proximity a bit of himself, his time, his attention, his considerations. My sincere prayers to his wife Jacque, his daughter Karen, and to the rest of the Sullivan family. This man was a hero.

I was one of his first patients, to ill to go to the office, and he came to our home. After three children and many years later we moved out of the area, but, never ever forgot Dr. John. He was "one of a kind" and will be sadly missed. Our prayers go out to the family.

The Sullivan Doctors were our family's physicians when we first moved to Chino in 1963. Dr. John had a way of making you forget you were even sick when you visited his office. From the Borges bunch...God Bless you all.
