Cieslak, Jan Born to Eternal Life Jan. 1, 2012, at the age of 90 years. Beloved husband of Anna. Loving father of Richard (Jane), Barbara (Dave) Hall, John (Terry), Diane (Jim) Voss, Ted and Irene (Mark) Kopplin. Dear grandfather of Angela (Steve), Scott (KD), Melissa (Darin), Steve (Megan), Chris, Erik, Brian, Sarah (Jeremiah), Jeff, Andrew and Joshua and great grandfather of Grace, Grayson, Jack and Noah. Further survived by family in Poland and other relatives and friends. Jan was a longtime member of the Basilica parish and was an Usher and former Choir member. Special thanks to the staff's of St. Luke Medical Center and Zilber Hospice for their kind and compassionate care. Memorials to the St. Josaphat Basilica Foundation would be appreciated. Funeral Friday, Jan. 6, at 12:30 PM from the Funeral Home. Mass of Christian Burial to be celebrated at 1:00 PM, at the BASILICA OF ST. JOSAPHAT, 601 W. Lincoln Ave. Entombment St. Adalbert Cemetery. Visitation Friday at the Funeral Home 10:30 AM-12:30 PM.

It was a pleasure to have Jan in our family. He was a good man and a great cousin. Whenever we had reunions it was always so much fun to be together and now he is experiencing a wonderful reunion in heaven. I will miss him very much and will keep all of you in my prayers daily.

I had the privilege of knowing Mr. Cieslak for more than 50 years, my dear friend, Ritchie's, dad. I remember him as hard working, devoted to his family, involved in his community, and a man of God. He was an example of that Polish-American spirit of the south side in its finest days. May heaven be his reward for a life well-lived and God's peace heal the hearts of his family.
