You have my deepest sympathy in the Loss of Dr. Holliday. I worked for Dr. Holliday in the late 60's, and he always stressed education. Dr. Holliday took me under his wing, and I went to school for surgical technologist. I am now a surgical tech in a Trauma hospital in Ft. Lauderdale. Dr. Holliday also taught me many things in life, and I will miss him for the rest of MY life. You have my prayers and may God continue to bless you

To the Family of Dr. Holliday:
Dr. Holliday was truly a visionary and well ahead of his time. When he contacted the Gary Model Cities Resident Committee and convinced them to fund the establishment of the Medical Center of Gary, few people knew that would be the beginning of a Health Maintenance Organization in Gary. He was at the forefront of the emphasis on bringing new medical talent to the City of Gary and that effort still continues to this day. He was a kind soul and his memory will live with me forever. May God bless his family.
Sincerely,
Victor & Hazel Thornton

To the Holliday family I extend my deepest sympathy and condolences.Al and I attended Indiana University School Of Medicine and later trained at Homer G. Phillips Hospital in St. Louis. He was ahead of me and a mentor. I even moved into the apartment he had in the Pruitt-Igoe projects when he moved to a better place. What I remember most was Al's attention to detail.
We lost touch and reconnected about 4 years ago and during a dinner, I was taken by his love and devotion to Iris. Al will be remembered as a doctor, educator, civil rights advocate, father and loyal friend. What more can you ask of a man than he be a man of commitment and character.
May his family find comfort in his works in life and his love for them.

To the family of "Doc" Holliday:
I have struggled with what to say to you since I learned of his passing. I am one of many Nurse Practitioners that he was responsible for training and if that was the only link to him that I had I would be forever greatful. However after finishing the nurse practioner program at I.U.N. I worked with him at the Lake County jail, till 1989, since that time I have been a solo provider in a Frontier Clinic in S.D., full filling a life long dream thanks to him. I have many fond memories of him and hardly a day passes that I do not think of him and count my blessings for having gotten to know him.
My prayers are with you in your time of sorrow!!!
Thank you for allowing him to touch so many people in so many different ways.

Holliday family and friends:
I have known of Doc's work in opioid addiction treatment since I joined the State of Indiana's Department of Mental Health Division of Addiction Services back in 1983, and Doc's reputation as a pioneer in this field and as a patient advocate was strong even back then. In the past year, I've had the opportunity to join the Office of the State Methadone Authority and to learn that Doc's work in this field continued and that his influence grew. I was fortunate to personally hear of his plans to develop an addiction center of excellence in Lake County and shared in his enthusiasm and hope for this project. I also attempted to support his avant-garde ideas to move northwest Indiana in the direction away from clinic-based opioid addiction treatment to pharmacy-based care for patients who had achieved medication stability.

My heartfelt condolences go to not only family members, but to the field of addiction and to the people of northwest Indiana who have and will continue to benefit from the legacy of Doc Holliday.

Doc Holliday was my neighbor for twelve years. The first day I moved in, Doc came right over and told me what I should and should not do. I just knew I had two Dad's! Until last summer, when my Dad passed and Doc's strength was ebbing, he made sure I could rely on he and his family for gardening, swimming, knowledge, the list is endless. He was very giving of his time, his expertise and his resources. My heart goes out to Iris, Ronny, Leroy, Dawn and those I did not have the opportunity to meet. I know that Doc and Dad are looking down, and keep on your toes because they are still looking out! You will be missed Doc Holliday. Melody, Mary and Jennifer Richards

To the Holliday Family,
My prayers are with your family during this time.
It is unfortunate that I never met
“Doc Holliday” in person but had several very meaningful conversations on the phone. He was not only a very special person who cared about everyone he met but he was full of visions of what each person he met could accomplish and how they could add something to the world that would make it a better place. It may sound strange to say that a man I never meet touched my heart in ways I didn’t think were possible but, he inspired me to take on challenges that I didn’t know if I had the strength to tackle. I will always be grateful for the words of wisdom he gave to me. I will continue to uphold “Doc’s” visions for the addicted persons that we serve.

Thank you “Doc Holliday" for all that you have done in your life. Your words, visions and life work with Methadone and addiction services will live on through all of us.

Dear Family of Doctor Holliday:
I was saddened, though not surprised, by the news of your loved one's passing. I pray for your family's strength as you go through the grieving process. As you do, remember that he touched a lot of lives, a lot of lives touched his and all were the better for it.
I got to know him ten years ago after I had joined the bureaucracy of state government and was assigned oversight duties of his "methadone" clinic. Over the years as I gained knowledge and expertise, some from his expertise, we became not only colleagues but friends. As colleagues we were joined in the fight to provide ethical, compassionate and respectful opiate addiction treatment services. And on a number of occasions he eventually sought my advice and guidance about how he might realize his dream.
Ecclesiastes 3 tells us that "to everything there is a season, and a time to every purpose under the heaven." Doc Holliday had his seasons and his purpose. I was fortunate to have been a small part of both. I valued him as a colleague, respected him as a man, and cherished him as a friend. I will miss him.

On behalf of myself and the entire staff here at of Semoran Treatment Center, I want to express our most heart felt condolences.

Doc, you are a great man who touched a lot of people in very profound ways.

I cherish the memory of the many drives we shared down to the state capital for the quarterly State Methadone Authority meetings.

You were always willing to freely pass on your hard earned wisdom to myself personally over the years and to a our patients and staff when you filled in as Acting Medical Director.

To honor your legacy as a pioneer in the field of medication assisted treatment we have renamed our clinic Communications Room the: Dr. Alfonso Holliday II Communications Room.

I have a lost a great friend, mentor and surrogate father may God take you into his eternal embrace.

To my dearest family, some things I'd like to say...
but first of all, to let you know, that I arrived okay.
I'm writing this from heaven. Here I dwell with God above.
Here, there's no more tears of sadness; here is just eternal love.

Please do not be unhappy just because I'm out of sight.
Remember that I'm with you every morning, noon and night.
That day I had to leave you when my life on earth was through,
God picked me up and hugged me and He said, "I welcome you."

It's good to have you back again; you were missed while you were gone.
As for your dearest family, they'll be here later on.
I need you here badly; you're part of my plan.
There's so much that we have to do, to help our mortal man."

God gave me a list of things, that he wished for me to do.
And foremost on the list, was to watch and care for you.
And when you lie in bed at night, the day's chores put to flight.
God and I are closest to you....in the middle of the night.

When you think of my life on earth, and all those loving years
because you are only human, they are bound to bring you tears.
But do not be afraid to cry; it does relieve the pain.
Remember there would be no flowers, unless there was some rain.

I wish that I could tell you all that God has planned.
But if I were to tell you, you wouldn't understand.
But one thing is for certain, though my life on earth is o'er.
I'm closer to you now, than I ever was before.

There are many rocky roads ahead of you and many hills to climb;
but together we can do it by taking one day at a time.
It was always my philosophy and I'd like it for you too...
that as you give unto the world, the world will give to you.

If you can help somebody who's in sorrow and pain,
then you can say to God at night......"My day was not in vain."
And now I am contented....that my life has been worthwhile,
knowing as I passed along the way, I made somebody smile.

So if you meet somebody who is sad and feeling low,
just lend a hand to pick him up, as on your way you go.
When you're walking down the street, and you've got me on your mind;
I'm walking in your footsteps only half a step behind.

And when it's time for you to go.... from that body to be free,
remember you're not going.....you're coming here to me.

My condolences to all the Holliday family. Doc was always a wonderful neighbor and friend. My fondest memory is of Doc always letting us swim in his pool but first making us clean the pool cover first. It usually was hot and took all day. Now that I am older I can appreciate the lesson he was teaching. Hard work leads to many rewards. Thanks Doc, for everything. I will miss you!!

To The Holliday Family,

Alfonso was my late husband's best friend. (Not to mention brother-in-law for a time too!)
Heaven may never be the same now that Alfonso Holliday and Julian Allen Jr. have been reunited. If two people would be able to cause havoc in heaven it would be these two.
No two men could be more proud of their families or their families more proud of them and their accomplishments.
Please know our hearts are with you and we are so glad we got to see him. I'm sure those two are raising hell right now.

To
the Holliday Family,
On behalf of the Scott family, I want to express our sincere
condolences to each of you. Thank you for sharing "DOC" with us.
As a fellow member of the Gary School Board, Doc taught me a lot, and
encouraged me to learn more about technology than I ever thought I
would know. It was a passion of his. He truly wanted to make sure our
children were more than well equipped with the lastest technological
innovations.
Because of his hard work and dedication as the Chair of the Technology
Committee, we were able to complete the application for the five
million dollar Challange Grant. We were able to complete the five year
technology plan required by the Indiana Department of Education in
order to receive an additional four million dollars. As well as many
otherr projects he took under his wings.
Thank you Doc for taking me along with you and allowing me to learn from you. It
has helped me to be better able to help our children. We love you.

The accolades that have been written have not even begun to scratch the surface of what Dr. Holliday has contributed to the field of methadone and addiction medicine. Doc will always be a legend in hundreds of lives that he saved. I met Doc in the 1970's when I was working at Gary's first drug program, Marona House. During this time he was helping the City of Gary in their efforts to open the first methadone program. Doc listened to his patients and believed them when they told them that they needed help. Because of that, patients came from all over Chicagoland and Indiana to seek out his services. I will continue to uphold Doc's name and carry out the vision he had for an all-addictions campus. I have been praying for Doc and his family and extend warm wishes to all.

Lately, people have asked me to share my most memorable and cherished moments spent with My Dad-Doc Holliday. Those moments were our regularly scheduled 'Father-Daughter Talks'no matter where he was in the world at special dates and times. During these times, we shared our Froebel High School,Indiana University-Bloomington and daily life experiences, but more than that recently we talked about my parenting skills to prepare me as my only child (my son) prepares for a life-threatening kidney transplant; with emphasis on the fact that he would not be here on earth to support me and for me to make a special attempt to accept in my grief process that My Dad-Doc Holliday would always be with me in 'spirit'.
I'll always love and cherish the days and nights My Dad-Doc Holliday and I spent creating the vision of Holliday Healthcare, P.C., especially when we wrote the training manual to certify the concept of the private out-patient methadone program; plus the days, week-ends and nights Dad, Kathy and I reviewed charts word-by-word by-the-law.
My Dad-Doc Holliday taught me to display the characteristics of a 'highly qualified' general education and special teacher by reading, writing, exploring 'the theory of teaching' everyday--focusing on 'if and then' projections. I was always proud when My Dad-Doc Holliday, the Gary School Board President would surprise me and sit-in on some of my classes at Wirt H.S. He made me especially happy when assisted me as a mentor to the Wirt H.S. Just Say No to Drugs Service Club programs. I thank God for My Dad-Doc Holliday allowing me to experience his gifts and talents 'side-by-side'working as an Indiana State Certified Addiction Therapist for 23 years at Holliday Healthcare, P.C.
Where Would I Be Today!? But, more than that-I will always be grateful for My Dad-Doc Holliday's insistence that I have soft-tissue surgery to the nerves in my right hand during my Christmas Vacation'05. This surgery performed by Gary native, certified general and plastic surgeon -Carl N. Williams,M.D.II of Las Vegas/Henderson, Nevada afforded me the opportunity to 'walk' without my cane or crutch before My Dad-Doc Holliday made his transition. There are many challenges My Dad-Doc Holliday has set before me,to include my present studies in Law School to become a Litigation Specialist upon my retirement from the Gary Community School Corp.--but,none greater than my 'courage' to recreate my life, even at my age of 56, through exercise and above all-daily prayers--to 'walk' before Friday, January 19, 2007.
I'll always love My Dad-Doc Holliday because he accepted me as his 'special daughter' just the way I am! The tears I cry are tears of 'joy' - My Dad-Doc Holliday is not suffering anymore.
I will work harder as a 'proud' Gary, Indiana teacher to show our children that 'education' is worth the struggle to be committed to attend class everyday-My Dad-Doc Holliday always stressed the fact that a high school diploma is only a part of the key to survive.
I, Thank You Dad For Never Leaving My Side!

To The Family of Dr. Alfonso David Holliday II,

With sincere condolence during your time of sorrow. Not only did I have the pleasure of having Dr. Holliday as my physician, I also went to school with his wonderful son, Alfonso Holliday III. Dr. Holliday was such a caring physician who truly cared about his patients. I was hospitalized with influenza in the 80's and you could not have told me he was not a miracle worker, for Dr. Holliday did everything, ran every test he could to find out what was going on with my illness. His bed-side manners were impeccable. He also impacted my life and encouraged me to get my education. I was a young mother and he assured me the road would not be easy, don't give up, be somebody and love your child like you've never loved before. Thank you Dr. Holliday. May you rest in peace, peace be still...
