JACK HOFFMAN
LIMA — Dr. Jack Roland Hoffman, 86, dedicated physician and cherished husband, father and grandpa passed away on Tuesday, August 4, 2015 at Lima Memorial Hospital in Lima, Ohio.
Born on August 19, 1928 in Crestline, Ohio to Theodore and Annabell (Hartman) Hoffman, Jack graduated from Capital University with a degree in Zoology and Chemistry in 1951. He received his medical degree from Ohio State University in 1955. After serving two years as a lieutenant in the U.S. Navy, he completed his residency in internal medicine at Milwaukee County Hospital in 1961.
Jack married his wife, June Armstrong, on September 5, 1954. They celebrated almost 61 years of marriage together. Jack was a devoted husband, father and grandfather, and enjoyed spending time with his family. He dedicated his life to medicine, practicing over 57 years. He was a co-founder of Medical Associates Health Care Center in Menomonee Falls, WI in 1961. In addition to his internal medicine practice, he served as the clinic administrator for over twenty years. After a brief retirement upon relocating to Sierra Vista, Arizona in 1986, he returned to his beloved medical profession in service for the non-profit Chiricahua Community Health Centers, Bisbee, AZ retiring at age 83. Medical Director for Cedar Lake Home, West Bend, WI, Jack also chaired or served on numerous boards including Board of Directors and Board of Appeals HMO; Cochise County Drug and Substance Abuse; Bisbee Foundation; and Bisbee Rotary Club. He was a clinical instructor for both Marquette Medical School and Medical College of Wisconsin. Jack was an avid reader and he and his wife enjoyed spending their summers on the farm in Wapakoneta, Ohio.
Dr. Hoffman is survived by his devoted and loving wife, June, and seven children: Jayne Hoffman of River Falls WI; Jeff (Julie) Hoffman of Santa Monica, CA; Jonn (Jane)Hoffman of Jackson, WI; Jackie (Bob) Dubois of Forest Lake, MN; Jodi Hoffman of Hastings, MN; Jenifer (Matt) Eichen of Winnetka, IL; and Jason (Debra) Hoffman of Phoenix, AZ. He is also survived by 14 grandchildren, a great-grandchild; sisters: Jean C. Hart of Winter Park, FL and Carol Ann Hardin of Jacksonville, FL and brother-in-law, Dean (Kate) Armstrong of Marion, KS; as well as many nieces and nephews.
He is preceded in death by his parents; his brother, Richard Hoffman and brother-in-law, Neil Armstrong.
Jack will be buried after a private memorial ceremony on August 8, 2015 in Greenlawn Cemetery, Wapakoneta, Ohio.
In lieu of flowers, donations can be made in memory of Jack to: Boys & Girls Club of Bisbee, AZ; Community Outreach Health Clinic, Community Memorial Hospital, Menomonee Falls, WI; or Arizona Medical Mobile Eye Unit.

How sorry and shocked I was to read of your passing in the AZ paper. It was a privilege to work for you at Medical asso. I'm sorry I flipped the first occupational health trailer. Ater that every time you would see me you would just shake your head and smile. Thanks for the warm memories.

june and family. jack was a great visionary. i think of him monthly for starting the new clinic insisting we allstart ani.r.a. etc. i had the chance to tell him thiese points and oyhers last summer not realizing it would be my only chance.. iwas lucky. he made much happen --i am grateful!! jack foley
