Jacob was born on July 9, 2012 and passed away on Thursday, March 14, 2019. Jacob was a resident of North Carolina at the time of passing. A funeral service will be conducted Sunday, March 17, 2019 at 2:00 PM in the J.C. Green and Sons Chapel in Thomasville with Minister Brock Shanks officiating. Interment will follow the service in Holly Hill Memorial Park. In lieu of flowers, please direct memorials to Brenner's Children's Hospital, Wake Forest Baptist Medical Center, Medical Center Boulevard, Winston-Salem, NC 27157.
