Beloved son of Hollie Zmuda and the late Brian Dienert.
Loving brother of Justin Dienert, Jordan and Savannah Willard.
Cherished grandson of Ken and Suzan Zmuda and the late William and Norma Dienert.
Godson of Sue Hawrylak and Kenny Zmuda.
Also survived by Aunts, Uncles, Cousins, a niece and many friends.
Family and friends received Thursday 2-4 & 6-8 PM at the Readshaw Funeral Home, Inc. 1503 Brownsville Road, Carrick where a Funeral Prayers will be held Friday at 10:00 AM.
Interment South Side Cemetery.
Jake always had a smile on his face, was a video gamer, loved his family, friends and was there for everyone.
