Jeremiah Moses Montoya-Peña,25, passed away unexpectedly on September 20, 2025. He was born on March 31, 2000 in Pueblo, CO to Sarah L. Peña and Tom Montoya. He was preceded in death by his mother, Sarah L. Peña; grandmother, Lorraine Sue LaChapelle; and cousins, X-avier "Nutsie" Padilla and Cameron "Kam Kush" Montoya.
Jeremiah had a deep love for music and was happiest spending time with his nephews and his dogs, Bebe and Prince. He was a devoted New England Patriots fan and enjoyed caring for animals. Above all, Jeremiah cherished being with his family and making memories with those he loved.
He is survived by his father, Tom Montoya; stepmother, Veronica Molina; sisters: Jazmin Montoya-Peña (Juan), Janyessa Peña (Michael), Jayna Peña, and Patricia Montoya; and his beloved nephews and niece: Juelz, Masyus, and Mahlani. He is also survived by his grandfathers, Brad LaChapelle and Thomas Montoya; grandmother, Elizabeth Roybal; aunt, Jerrilee Peña; goddaughter, Cataleya Sandoval; cousins, Chaz Peña, Maribelle Sandoval, and Rome Arguello; along with many other extended family members and friends who will forever hold Jeremiah in their hearts.

Funeral Service, 11 am, Thurs., October 2, 2025 at Victory Outreach Church
