Preceded in death by grandparents: Anthony and Catherine Couvillon, Howard Poly; aunt Elaine Trent. Survived by parents, Dennis and Lynn Poly; sisters, Marie and Ellen Poly; grandmother Louise Poly. Loved by numerous aunts and uncles, cousins and friends.
VISITATION Sunday 3-5pm, St. Bernadette Catholic Church, 7600 S. 42 St., with VIGIL SERVICE at 5pm. MASS of CHRISTIAN BURIAL Monday 10:30am, St. Bernadette Catholic Church. Interment, Calvary Cemetery. In lieu of flowers, Memorials may be directed to the family for charities of their choice. HEAFEY-HEAFEY-HOFFMANN

Lynn and family, I just found out about your tragic loss and my thoughts and prayers go out to you. I remember how you used to share your "Daniel" stories with us everyday at lunch. We got such a big kick hearing about his latest escapades and cheered with him every time his teacher gave him a smiley face for the day. He brought such joy to your family during his short life. Hold on to those memories in the days and years to come.

My prayers go out to you.I knew Dan in highschool.I was a new transfer student and the only girl in a strength and conditioning class full of guys.he was such a joy to be around,always smiling and encouraging me to run a full lap around the track.although what he did might have seemed little,it meant a lot,and everytime I saw him,he was smiling or laughing.May God give you peace and comfort through this difficult time.you are all in my prayers.

When I first heard, I was stunned and shocked. I know how hard it is. I am deeply sorry for the death of Dan. He honestly was a wonderful person at heart. I am honored to have known him from middle school, highschool and beyond. I wish I could have seen him since highschool but that is the past. May you find peace; truly.
