I am so sorry for your loss. I joined Chuck's team at KP one year ago. Chuck's gentle manner was a gift to us all. It gives me peace when I see our providers practice what Chuck has taught us. Our patients are safer today. Chuck I miss you, Nancy told me you said you are in a good place. I am thankful to have been part of your team.

I will never forget every summer spent with uncle Chuck...driving up and down the west coast, camping trips, Disneyland, and beach house. He taught me so many valuable life lessons and I would be happy to be half the person he was. You will be dearly missed, Uncle. I love you.

I had the pleasure of working with Chuck at Kaiser Roseville and his calm demeanor, humor in the face of all adversity and his passion for TCAB stick clearly in my memory. My thoughts and prayers are with his friends and family right now.

I feel honored to have known Chuck for 14 years from the time we worked together at UCDMC and again at Kaiser Permanente.

Chuck possessed a rare combintation of professionalism, humor and his passion for patient safety. Those of us who knew Chuck professionaly will continue to be proud of the excellent work he did in improving patient outcomes. He wanted to make a difference and he did-not for personal recognition but because it was the right thing to do for the patient.

I will also always remember him a true friend who helped me and our colleague Beth get through some of the rigors of our often difficult job with a lot of laughter. We were a team in the true sense of the word! Those days will live in my heart forever. He also inspired me to pursue my Master's degree and when I wanted to give up, he motivated me to keep going.

My deepest condolences to Fay, Christopher and Melissa. Everytime he spoke of you his eyes would light up and he would get that Chuck smile- a smile that will be missed forever.
