Born: March 10, 1963 in Melrose Park, IL
Died: February 26, 2024 in Dixon, IL
Thomas Michael Dziuban, born March 10, 1963, passed February 26, 2024. Tom was preceded in death by his mother, Anita Elizabeth and father George John. He is survived by his three siblings, David George, Mark Anthony and Georgann Kennay, numerous nieces, nephews, great nieces and great nephews. Tom was fun loving and enjoyed being with others. He will be best remembered for his wit and practical jokes. He enjoyed sports of all sorts and would often attend local park district sporting events and high school football games. He moved to Kreider's residential program in 1984 eventually moving to Kilker apartments managed by Kreider Services, Inc in 1996 where he resided until death. He worked at Kreider Day Services for over 39 years. His family extended beyond his immediate family and included those who supported him within the Kreider family, most importantly his former care provider Kenny Blair and current caregiver Roberta Considine. In lieu of flowers, the family asks for donations directed to Kreider Services, Inc, Kilker Apartments in Tom's name. Arrangements by the Jones Funeral Home in Dixon.
