John Horvath Obituary

John L. Horvath, MD July 6, 2015 CARMEL - John L. Horvath, MD, 74, of Carmel, Indiana, the loving father, grandfather, brother and dedicated physician passed away peacefully in his sleep the morning of July 6th with family by his side. "Jack" grew up in South Bend, Indiana and was a graduate of Riley High School. He was a home town boy and attended his beloved University of Notre Dame and was a true dedicated "Domer" for life. Jack went on to Indiana University Medical School and graduated in 1966. He served as a Captain in the Air Force during the Vietnam conflict. He completed his residency in Radiation Oncology at IU Med School. From there he went on to practice in Radiation Oncology at St. Elizabeth Hospital in Lafayette and then Memorial Hospital in South Bend. In 1979 Jack and his family moved down to Carmel where he practiced Radiation Oncology with St. Vincent for 27 years. In 2006 Jack was honored with the Distinguished Physician Award from St. Vincent. He tried retirement for a brief time in 2006 and found it just wasn't for him and he joined Witham Health as their Chief Medical Officer where he continued working until he passed away. Over the years Jack treated many patients who came to love his empathy and caring during their time seeing him. He was equally as loved by all of the dedicated nurses and physicians who had the privilege of working with him. Outside of work Jack loved to play golf and had many great times golfing with friends and family over the years. He was a long time member at Crooked Stick Golf Club where he enjoyed countless rounds of golf as well as many wonderful times with family and friends at the club house. Jack also traveled the world to play golf and some of his favorite rounds included St. Andrews in Scotland and Pebble Beach in California. Jack was preceded in death by his wife of 45 years, Susan Horvath as well as his parents, John and Clementine (Lesniewicz) Horvath of South Bend. He is survived by his daughter, Lisa Horvath (Denver, CO), daughter, Kim Hall (Carmel) and son, Andrew Horvath (Denver, CO); as well as his beloved grandsons, Jacob Horvath, Carson and Cody Hall and Max Horvath; and brother, Dr. George Horvath (Paula) of South Bend, and sister, Kathy Foley (Jerry) of Valrico, FL and countless other friends and loved ones. In lieu of flowers Jack would like to have donations made to two of his favorite organizations: The Gennesaret Free Clinic of Indianapolis and the Witham Foundation. The calling will be Thursday evening from 4-8 pm at Leppert Mortuary in Nora and funeral services will be Friday morning at 11 am at Our Lady of Mount Carmel, Carmel, IN.

I worked with Dr, Horvath for 10 years at St.Vincent Radiation Oncology. I will always remember him as such a loving, caring person, not only to his patients,but to his staff. He cared so much. I learned so much just being in his presence. I feel so blessed for having the years I worked with him.

Dr George and family, so sorry for the loss of your brother. Phyllis (Johnson) Herzog has shared fond memories of all of you and I have a few of my own when I was over playing with Laurie. May the Lord and His angels look over you during this time and always.
