Patrick Sample Obituary
News Death Notice
SAMPLE, Patrick A. 61, of Springfield, passed away on Sunday, February 21, 2016 in Springfield Regional Medical Center. Pat was born May 20, 1954 in Springfield the son of Joseph E., Sr. and Bette Jo (Reiter) Sample. He had been employed at Foreman-Blair for many years before retiring and later worked in the Pro Shop at Windy Knoll Golf Club. He was the golf coach at Catholic Central High School for 15 years and a member of St. Raphael Catholic Church. Pat was the recipient of a successful organ transplant this past August and his family encourages others to donate life. Survivors, in addition to his mother of Springfield, include his wife, Cheryl (Gladman); two daughters, Amy (Chris) Fralick of Springfield, Anne (Zack) Hines of New Albany and their mother, Laura Dancy; two step-children, Heather Baker of Springfield and Nicholas Rapp of Troy; three siblings, Mike (Kaye) Sample and Mark Sample both of Springfield, Lisa (Tim) Bowman of Columbus; six grandchildren, Will and Sammy Fralick, Josey and Stella Hines, Taylor Baker and Dominik Rapp. He was preceded in death by his father in 1999 and his brother, Joseph E. Sample, Jr. in 1998. Friends may call in the CONROY FUNERAL HOME from 4:00 p.m.-7:00 p.m. on Thursday. Mass of Christian Burial will be held at 10:00 a.m. Friday in St. Raphael Church. Burial will be held in St. Bernard Cemetery. Memorial donations may be made to Catholic Central High School.

To the Sample family my deep and sincere condolences...Pat was a long time friend. We did not speak often however when we saw each other it was like we were back at St Raphael's. I know he will be missed...May God Bless the family during this difficult time!
