37, born 10/16/1973, passed away on 5/25/2011. He is survived by his wife Terri, his children David, Chelsey and Joshua, his parents, sister Melissa and brother Tony. Services will be held Tuesday 6/7/2011 from 1:00 – 4:00 at La Paz Funeral Home 935 E. Indian School Road, Phoenix AZ.
