Jacob was born on June 21, 1989 and passed away on Saturday, August 4, 2018. Jacob was a resident of Kentucky at the time of passing. Visitation will be held on Sunday, August 12, 2018 from 4pm until 7pm at Floral Hills Funeral Home in Taylor Mill, KY. Funeral Services will be held at Floral Hills Funeral Home on Monday, August 13, 2018 at 10am.
