Jacob Cole Slade, "For The Love of Baseball", 12 of Mesa, AZ passed away on July 17, 2004 in Tucson, AZ. He was born November 19, 1991 in Mesa, AZ. He is preceded in death by his grandmother, Delores. He is survived by grandparents, Wayne and Sue Slade and Joseph and Mary Lou Couey. He is survived by his parents, Darin and Kelly; brothers, Joseph and Joshua; sisters, Ciera, DeAnn, Rylee and Jocelyn; aunts and uncles, Dennis and Karen Slade, Darwin and Marcy Slade, Darla and Steve Mitchell, Dena and Roger Paeselt, Darlene and Dave Osborn, and Daniel Slade; cousins, Callie, Austin, Morgin and Levi Slade, Sheridan, Tatum, Colton, and Taya Slade, Lexie, Kylie and Blake Mitchell, Amanda, Ashley, Alexander and Alison Peaselt, Emeline and Heidi Osborn, and Chase Sparks Slade. Visitation will be held on Wednesday July 21, 2004 from 6-8:00 p.m., Bunker's Garden Chapel, 33 N. Centennial Way, Mesa, AZ. Funeral service will be held on Thursday July 22, 2004 at 10:00 a.m., LDS Lehi 9th Ward, 830 E. Lehi Rd., Mesa, AZ with visitation one-hour prior to services. Jacob was trustworthy, loyal, helpful, friendly, courteous, kind, obedient, cheerful, thrifty, brave, clean and reverent. Although I didn't know Jacob, I had Joey as a student for three years at Stapley Jr. High and just wanted to tell you how sorry I am for this tragic loss. You are a wonderful family and I am glad you have each other at this time. Take care and God Bless.

It was an honor to know Jacob and to be able to associate with your family. I have many wonderful memories of Jake. He was always happy and energetic and lived life to its fullest. He was fun to be around and was kind to everyone. Jake taught me a lot too. He was always patient and gentle with his brothers and sisters. Being with Jake was always a good time! I love him and your family and will miss him tremendously. My thoughts and prayers are with your family.

i am truely sorry for your loss. jacob was always smiling no matter what happened. he was always a great friend and was always there to cheer anyone up. he always wanted to make more friends and helped anyone who was down but was still there for his other friends. he was a great baseball player and yet still a good horseback rider. your family is in my prayers and if you guys need anything i am here for you so just give a call.

We had just gotten back from vacation to hear a very sad, and shocking message from a mutual friend of ours, telling us of the heartbreaking news. We have such fun and sweet memories of the Slade family as they have grown. Some people leave a mark and fill a place in our lives, regardless of the limits in time we have to spend together. The Slades have made such a mark and filled such a place in our lives. Jacob has made such a mark and filled such a place. We have watched him grow from an infant still in his hospital crib, to a toddler on the beach, a boy on a horse, and a fine young man.
