It's taken me a long time to reconcile losing such a great physician and wonderful mentor. You saved my father's life more than once. You guided me and mentored me into becoming the physician that I am today. Make no mistake, without Dr. Van Dyken allowing me the privilege to shadow him in the office and go on rounds with him at Chilton, I don't get into medical school. He was the guiding inspiration. Iam so sorry to hear about Dr.Jacob i work at chilton and would see him all the time while i was transporting patients and he would always say to me i wish i had what u are taking meaning i was always smiling and he would call me smiley all the time. He was a great doctor always friendly and he will be missed, my heart goes out to the family,

I remember how he always came in to the Hospital early each morning through our (Chilton's) ER. He always was so pleasant, had a thoughful word with all of us, and was always so appreciative of anything anybody did for him. We'll greatly miss him and his smiling face.

You took good care of my parents and you've been taking good care of me. I'm stunned and deeply saddened. I will always remember you and would like to say goodbye to you the way we always parted... Peace my man...

I have wonderful memories of Gail and Jake being soul mates even in high school! My heart goes out to the entire family, with love and sadness for this tragic loss. Rest in glorious peace, Jake!

Dr. Van Dyken was the greatest. He was absolutely wonderful to my Mother & was great to me. I will turely miss him.
