Maya Elizabeth Lewandowski passed away Tuesday, October 23, 2012 at Christ Hospital in Oak Lawn, IL. Baby Maya is survived by her parents Adam and Amy Lewandowski; two sisters, Emma Faith and Olivia Lynn; one brother, Luke Witold.
