Michael "Mike" E. Forman

Michael “Mike” Edward Forman, 65, requested a Viking Burial. In honor of Father’s Day & his untimely passing on June 14, his wife & children are now obligated to fulfill his request. Born November 11, 1953 Mike was a Chicagoan in heart & soul. Always a dreamer & natural leader, he lived life with a passion for family, football, & travel. Born to Thomas E & Frances M Forman, brother to Tom, we assume they’re now all sharing a drink together. He is survived by his loving wife, Tami, whom he respected & trusted more than anyone on this Earth. His children, Jess (34), Nick (29), Danni (25), Alex (24), Rachel (21) & grandchildren and beloved dogs. We will always remember his words of wisdom & advice on every subject under the sun. A memorial service will be held in his honor at his home away from home, Crossroads of Ivanhoe, 20915 Park Ave, Mundelein on Friday 6/22 12-3pm. Come out to enjoy good laughs, memories, & friends; the way Mike always wanted it to be. In lieu of flowers we have set up a junior football scholarship fund in his honor.
