Richard J. Siwek, 65 of Phoenix, AZ, passed away September 24, 2006. Richard was born Februay 6, 1941 in Chicago Illinois. He is survived by his son Richard G Siwek, daughter Susan Siwek, both of Florida,brothers Raymond of Arkansas and Joseph of Illinois. A private service will be held.
