May 8, 2007. Beloved husband of Joanne (Short) Forman. Residence: 6 Westford Rd., Providence. Dear father of Roger Forman and his wife Gwen of Walpole, Guy Forman and his wife Pamela of Needham and Deborah Forman and her husband Matthew Finger of Providence. Devoted grandfather of Brianne, Mack, Lucy, Charlotte, Julia, Caroline & Nathaniel. Loving brother of Audrey Robbins of Providence. He was the brother of the late Jill Forman Starr. Funeral service 10:00 AM Thursday, May 10th, at Temple Beth El, 70 Orchard Ave., Providence, RI. In lieu of flowers contributions may be made to Temple Beth El or Miriam Hospital, 164 Summit Ave., Providence, RI 02906. Shiva will be observed at his late residence Thurs 5-8 and Fri 2-4 PM. The family was assisted with the arrangements by Sugarman-Sinai Memorial Chapel.
