Ivan Vazquez Obituary

We are sad to announce the death of Ivan H Vazquez MD, age 96. He died on January 11, 2023 surrounded by his loving family at Northwest Community Hospital. Ivan, son of Oscar and Laura Maldonado Vazquez, was born in Ponce, Puerto Rico. He was the youngest of six siblings, Maria, Hector, Salvador, Antonio and José Raphael. He left Puerto Rico at age 18 to attend and eventually receive his medical degree from Marquette University. His accomplished career includes serving as a Captain in the US Amy Medical Service Corps for 8 years. He was a founding physician at Northwest Community Hospital in Arlington Heights. He was also one of the first doctors to open a private practice in Hoffman Estates. He was the beloved father to Jean (Rodney) Domenech, the late Peter Vazquez, Ann (Jim) Miller, Michael (Cindy) Vazquez, Nancy (John) Erdmann and Patty (Stevan) Pavlovich. He has 15 loving grandchildren and 9 great-grandchildren. He is also survived by his cherished partner Linde Wentling. The funeral will be a private ceremony with close family members only. We invite you to make a donation to JourneyCare Hospice or Northwest Community Hospital in his honor, in place of sending flowers.

Dr. Vazquez was our family doctor starting in the 1950's. We lived in Parcel A and his office was a home in Parcel B. What a wonderful doctor. I am so sorry for your loss.

I remember Dr Vazquez as our doctor back in the 50's when he opened his practice in a house on Roselle Road in Hoffman Estates. So many years ago! An original "country" doctor to all of us in HE.

He was a great person and dr. He would come
to the house, when we were sick and we had 1 car only. we still talk about him these days. I plowed his parking lot around 1968.
