Jack was a very good friend of my cousins Fred and Chris Mangialardi.I believe Fred and Jack attended St. Philip High School, next to Our Lady of Sorrows School, where I went to school for first and second grades. I remember seeing Jack and my cousins at the Jackson Boulevard home in Chicago. After I secured a teaching position at Barrington High School, I lived in Wauconda. Being ill one day, I entered Jack"s office and we quickly connected to our families' roots. From that time on, I saw him as my family physician. I am greatly saddened by his passing. He helped me and my family immensely and held the utmost of our confidences as any physician should. I shall truly miss him and trust that his family will find condolence s in his service to his patients and to his community. Thanks Jack.

I was Jack's classmate at Loyola and I remember Jack taking me to his father's office when I had my hyperventilation syndrome in medical school. I had an EKG taken and was reassured that I was okay. Jack was quite a character then and I am very saddened to learn of his passing.

I am Jack's first cousin, daughter of his uncle Bruno Bellucci. Although many years have passed since I have seen my cousin Jack I am truly saddened to hear of his passing. I pray that each and everyone of his children and their families, his brother Frank be blessed with God's peace and comfort in their time of sorrow.
A mass will be said for Jack at St Michaels Catholic Church here in Bedford, Texas. May he rest in God's peace.
Once again my deepest sympathy.
